I just ordered this book last week (still waiting for it in the mail). I'll let you know how I like it. I've heard good things about it though!

My Therapist let me use it while I was counseling with her.
It was a really good book and gave me a lot to think about.

I worked through this workbook week by week with my dietician, and although I complained about having to do it, I did find it helpful and also insightful. It really helped me to look at what it is about myself that I am self-conscious about, whatparts of myself are worth worrying about (some things are just the way you are and you can't change them, whereas some things are a quick fix - like getting a new haircut). It was good to go over it week by week with my dietician because she brough me back to reality and wouldn't let me rationalize my ED thoughts. Good luck with it!
